Ultimate Care consists of a dedicated team of community connectors whose primary duty is to link individuals with intellectual or developmental disabilities (IDDs) to resources, education, and support services available within their communities. Our connectors also help people with IDDs utilize community host homes and participate actively in community-based activities. Ultimately, the goal of this assistance is to help others maximize the opportunities their community offers them while promoting the value of socialization in a person’s day-to-day living.
